Diphenoxylate/Atropine (Lomotil, Lonox, Lomanate, Logen)

Description:

• Antidiarrheal: DEA Schedule V

Indications:

• Diarrhea

• Reduction of ileostomy discharge

Contraindications:

• Obstructive jaundice

• Diarrhea associated with pseudomembranous enterocolitis or entertoxin-producing bacteria

Precautions:

• Pregnancy category C; excreted in breast milk

• Acute ulcerative colitis (may induce toxic megacolon)

• Severe hepatorenal disease

Adverse Reactions (Side Effects):

• Central Nervous System: fatigue, dizziness, drowsiness

• Gastrointestinal: abdominal pain or distention, constipation, ileus, nausea, vomiting

• Dry Mouth

• Rash

Dosage:

Administered orally (tablets, solution)

• Tablet: 2.5mg diphenoxylate/0.025mg atropine 

• Solution: 2.5mg diphenoxylate/0.025mg atropine per 5ml
  

• Adult: 5mg (2 tablets) four times daily, taper dose as tolerated

• Child: 0.3-0.4mg/kg daily in 4 divided doses
